Please select the "8" key on your phone following each dictation and then make sure you give the following required information:  SAFE Case ID, SAFE Worker ID, Activity Date, Activity Time, Activity Duration and Activity Type

WORKERS BEWARE!  

Passwords expire "to the minute" 90 days after your password was last changed.  If you're in the middle of a referral or document and that "minute" comes up, you will lose any data not saved (you have to get out of SAFE - NOT saving changes and call the Helpdesk to have a new password created).   PLEASE make sure you change your password BEFORE the day it tells you it expires.

SAFE Password Requirements

Passwords must have the following

Between 8 and 30 characters

At least one upper case character

At least one lower case character

At least one numeric character

Ansell-Casey Life Skills Assessment (ACLSA)

ACLSA Codes:
        Salt Lake Region = UTDCFS18        

Northern Region = UTDCFSN

     Eastern Region = UTDCFSE        

Western Region = UTDCFSW 
Southwest Region = UTDCFSSW

Youth ID = Seven-digit SAFE ID

The youth and caregiver(s) must use the same Youth ID and enter the same email address in the "Matching E-mail Address" section on the Login Information page. The caseworker’s email will be used in the “Matching E-mail Address.” The Matching Score Report is emailed to the caseworker immediately after the caregiver(s) and youth complete the assessment. The Score Report is sent in the email as a PDF attachment.
The caregiver’s assessment and youth’s assessment must be completed and entered on the ACLSA website within 42 days of each other or the ACLSA website will be unable to match the youth and caregiver’s.
